Welcome to the Montgomery High School Clinic
Welcome to the Montgomery High School Clinic. My name is Shelley Webb R.N. and I look foward to an exciting year.
I want to welcome Susan Slavens to the clinic as my assistant. She is a great asset to the clinic.

MEDICATION POLICY
All medications brought to the clinic must be in the original container, clearly labeled with the student's name, and accompanied by written parental permission. Over the counter medications must be dispensed in accordance with the labeling directions. Over the counter or non-prescription medications needed for longer than two weeks must have a written request from a physician.

All prescription medications must be kept in the clinic in the original container with a complete and accurate prescription label. When a dosage change is necessary, the request must come from the doctor as well as the parent. All prescription medication must be transported to and from school by a parent or guardian. Students are not allowed to carry prescription medication on campus.


Required Immunizations
Student immunizations must be current in order for a student to attend classes. You will receive written notification if your child is due for an immunization. Please address this immediately so that your child's education can proceed uninterrupted. All immunization records are kept in the clinic.


DPT/DTap/TD
Three doses including one received on or after the fourth birthday and one dose within the past 10 years.


POLIO
Three doses with at least one dose on or after the fourth birthday.


MEASLES
Two total doses with the first dose on or after the first birthday. The second dose must be given no later than the 12th birthday.


RUBELLA
One dose on or after the first birthday.


MUMPS
One dose on or after the first birthday.


VARICELLA (Chicken Pox)
One dose of varicella vaccine on or after the first birthday is required for grades K-4 and 7-12. Students beginning varicella vaccine at age 13 or older are required to have 2 doses. This requirement can also be met by submitting serologic proof of immunity or documentation of previous chickenpox disease.


Hepatitis B
Three doses of hepatitis B vaccine are required for students in grades K-5 and 7-11.



The school nurse will monitor and require immunizations up to 6 months before deliquent date.
